WINTER SPORTS NOTICE

 

Please be aware that any student who wishes to participate in a sport at Central Middle School must have a physical within 365 days prior to the first tryout session.  Winter sports including girls and boys basketball, wrestling, and cheerleading, are scheduled to begin on November 12, 2018, therefore, a physical completed any time after November 12, 2017 would be acceptable. According to New Jersey state law, the physical must be recorded on a specific state form and kept on record at Central Middle School’s health office.  Please return completed forms to the school no later than November 2, 2018 to avoid any problems in your child being able to participate.  Physical forms may be downloaded from this web page.
  

***** Please drop PHYSICAL FORMS off in the main office during the summer to avoid any delay in your child trying out.  *****

 

ONLY PHYSICAL FORMS WILL BE ACCEPTED DURING THE SUMMER BREAK

 

To check the date of your child's last physical exam on file, log on to Genesis and click the Documents tab, then click "Physical exam status notification."

STUDENTS NOT PARTICIPATING IN SPORTS

Throughout the year individual students may see their own private physician for physicals. It would be most beneficial to have the records of both the physical and any immunizations received kept on file in the health office even if the student does not intend to participate in a sport.
